ACHC requires that all staff receive training appropriate to their role before providing services and on an ongoing basis. Training must be documented, and staff must demonstrate competency — not just attendance.

Training documentation is as important as the training itself. ACHC surveyors will review training records for every staff member. Missing or incomplete documentation is treated the same as no training at all.
Training Logs
Date, topic, trainer name, duration, and method of delivery for every training session.
Attendance Records
Signed attendance sheets or electronic confirmation for each participant.
Competency Evaluations
Written or skills-based assessments demonstrating that staff can apply what they learned.
Staff Acknowledgments
Signed acknowledgments that staff have read and understood relevant policies covered in training.
03. Preparing Staff for Surveyor Interviews
ACHC surveyors routinely interview staff — clinical, administrative, and management — to assess their knowledge of standards and agency policies. Staff who are unprepared or give inconsistent answers are a major red flag.
